Unpaid claims and slow payments hurt any practice's cash flow. We follow up on unpaid and underpaid insurance claims, fix what's holding them up and appeal when needed, before the payer's deadline passes.
Older claims take persistence. Our goal is that no claim is written off while it can still be paid. Here's how we work them:

Why age matters. Every payer has filing and appeal deadlines. For Original Medicare, a claim must be filed within one calendar year of the date of service, and a redetermination (first-level appeal) must be requested within 120 days of receiving the initial determination. Commercial payers set their own limits, often shorter. We work claims by deadline, not just by age.

What changed in 2026 (CMS). For services on or after January 1, 2026, Medicare pays physician services using two conversion factors: $33.4009 for most clinicians and $33.5675 for qualifying Advanced APM participants, up from $32.3465 in 2025. That changes what a correct 2026 Medicare payment looks like, so accounts receivable follow-up should compare payments with the 2026 rates, not last year's.

Claims shouldn't sit unattended. We track each open claim and follow up with the payer to check its status and fix what's holding it up, such as missing information or processing errors. Steady follow-up keeps claims moving and helps bring down days in A/R.

Outsourcing accounts receivable management gives you specialized expertise and consistent follow-ups without increasing internal workload. What you get:
Claims get worked on a schedule, not when someone has a free hour.
Payer calls, portal checks and appeals come off your team's desk.
They fix the coding errors behind unpaid claims instead of just resubmitting them, and they handle the payer follow-up.
See open claims by payer, provider, location and CPT.
Recovering old and complex claims requires more than routine follow-ups. It requires a strategic and data-driven approach. We review each older claim for denial reasons, documentation gaps or payer issues, then rework, resubmit or appeal it. We start with high-value claims and those nearest a deadline, so less revenue is written off.
Accounts receivable work goes better when it's tied to billing, coding and denial management. When follow-up finds a coding or billing error, we fix the cause so the same problem doesn't come back. See Denial Management and Revenue Cycle Management.
The right partner makes a real difference to how much of what you bill you actually collect.
We track and follow up on each open claim. Our AAPC/CPC Certified Coders and Billers do this work in the EHR/PM system you already use, and you can see where every open claim stands.

Reports break unpaid balances down by payer, provider, location and CPT, so you can see where they're aging. When the reports show a bottleneck, such as one payer slowing down or one provider's claims coming back, we look for the cause and work on it.
