Denied claims, delayed revenue, and increased administrative workload impact financial performance. Without a structured approach, denied claims sit unworked until the appeal deadline passes. We find the root cause of each denial, rework or appeal the claim, and fix the problem upstream so it happens less often.
We cover each stage, from the first look at a denial to the appeal.
We review denied claims to find patterns and root causes, grouped by payer, provider, location and CPT, so you can see where repeat denials start.
We correct denied claims to the payer's requirements, fixing coding or adding documentation before we resubmit.
A structured appeal management process is followed to challenge denied claims. We prepare appeals with supporting documentation and submit them within the payer's appeal deadline.
We put preventive measures in place to reduce future claim denials. We also tighten billing workflows and keep our team current on payer requirements to support clean claim submission.
Reducing denials takes prevention, not just rework. We work on:

Managing denials across several providers and specialties is harder. We track denials for the whole practice and report them by payer, provider, location and CPT, so you can see whether a problem sits with one provider or across the practice.
What changed in 2026 (CMS). Since January 1, 2026, Medicare Advantage plans, Medicaid and CHIP programs and their managed care plans must give a specific reason when they deny a prior authorization request (CMS Interoperability and Prior Authorization Final Rule, CMS-0057-F, released January 17, 2024). That makes it easier to fix and resubmit, or appeal, authorization denials.
Deadlines still matter. For Original Medicare, you have 120 days from receiving the initial determination to request a redetermination (the first level of appeal), and the claim itself has to be filed within 12 months of the date of service. Commercial payers set their own limits, and we track them by payer.

A proactive approach to denial analysis is essential for achieving long-term billing success. We track denial trends and dig into root causes such as coding errors, eligibility problems and documentation gaps. Then we fix them at the source, so the same denial is less likely to come back.
We report denials by payer, provider, location and CPT. That shows you why claims are getting denied and where improvements can be made, so you can make better decisions about your revenue cycle.

Unresolved denials quietly drain a practice. Tell us your denial volume and the payers that cause the most trouble, and we'll show you how we'd work them.

Contact usClaim denial management is the process of identifying, analyzing, and resolving denied insurance claims so healthcare providers receive proper reimbursement. We work each denial and look for its root cause.
Insurance claims can be denied for several reasons, including coding errors, missing patient information, eligibility issues, and lack of prior authorization. We focus on identifying these issues early and preventing recurring denials.
Working denials promptly brings in revenue that would otherwise be written off, and fixing root causes means fewer denials later. We handle claim resubmissions and appeals with the corrections each payer asks for.
Our AAPC/CPC Certified Coders and Billers work your denials inside your existing EHR/PM system, with regular reporting on what's being denied and why. We're HIPAA compliant, and you keep ownership of your data.
