Even small documentation gaps can mean lost revenue, delayed payment or compliance risk. We review encounter documentation, capture each billable service, enter the charges and reconcile them against what was scheduled and documented, so missed, duplicate and late charges get caught before the claim goes out.
Here's how we capture and enter charges, step by step.
We start by reviewing the clinical documentation for each encounter, procedure and service, and flag anything incomplete.
Once we verify documentation, we identify and extract all billable services, including procedures, treatments and ancillary services that are easy to miss.
We enter each charge in your EHR/PM system the same way every time.
Our AAPC/CPC Certified Coders and Billers check each charge's codes and modifiers against the documentation and payer rules.
Then we reconcile charges against the schedule and documentation. This step helps identify missing, duplicate, or inconsistent entries.
At this stage, we align charges with current payer contracts. We flag fee schedule entries that are out of date or don't match your contracts, for your approval before anything changes.
Once the checks are done, we post the charges promptly, so claims aren't held up.
Before claims go out, we audit the documentation, codes and posted charges against current payer rules.
The final step involves detailed reporting and performance tracking. You get reports by payer, provider, location and CPT, so you can see where charge capture gaps keep showing up.
Charges go straight into the EHR/PM system you already use, and the data stays yours.

Charges usually go missing in the same places: services documented but never entered, ancillary services and supplies left off, late documentation, and charges entered after the claim already went out. We reconcile encounters against the charges entered, so these get caught. Late charges matter: Medicare generally won't pay a claim filed more than one calendar year after the date of service.

We use the same steps for every encounter, check documentation before entering charges, and check again before claims go out. Errors are easier and cheaper to fix early.

Contact usCharge capture makes sure services provided to patients are recorded, coded and billed. It helps prevent lost revenue from services that were performed but never charged.
Charges that are missing, wrong or late either aren't paid, are paid incorrectly or are paid later. Correct, prompt charge entry means fewer denials and fewer corrected claims.
Charge reconciliation in medical billing is the process of comparing recorded services with submitted claims to check that all charges are accounted for. It helps identify missing, duplicate, or incorrect entries, improving billing accuracy and compliance.
It takes charge entry off your staff's desks. It also gives you AAPC/CPC Certified Coders and Billers without hiring.
Reconcile charges against schedules and documentation, train staff on documentation, and review charges regularly. These steps help reduce errors and missed charges.
